Patients are responsible for payment at time
of service. Please remember that your insurance policy is
a contract between you and the insurance provider; therefore,
the clinic cannot guarantee payment of your claim and must
render the patient responsible for any services not covered
by their insurance.

Prescription Refill Request
Prescriptions refills can be submitted 24 hours a day. Online prescription requests are monitored 8:00a.m. to 4:00 p.m., Monday through Friday.
