Financial Policy & Insurance
Patients are responsible for payment at time of service. Please remember that your insurance policy is a contract between you and the insurance provider; therefore, the clinic cannot guarantee payment of your claim and must render the patient responsible for any services not covered by their insurance.

What you should bring to your appointment
In order for your physician to better serve you and provide the best possible care, please bring the following items to your appointment:
- If you have insurance, please bring your Health Plan Insurance card
- Any necessary or related medical records
- A list of the medications you are taking, including dosage and the way you are taking them.
- A referral from your primary care physician if your insurance requires.
- Any related x-rays, ultrasounds, CT scans or test results.
